proMedico (Business) Claim Process
Each claim is different, let us guide you through your claim:
Fill up Liberty’s medical claim form
Please fill in medical claim form and mail the original *supporting documents to Liberty at One Raffles Quay #25-01 North Tower Singapore 048583.
*Supporting documents
- For Private Hospital:
- Insured to complete page 1 and 2 of the Medical Claim Form
- Attending physician/specialist to complete page 3 and 4 of the medical claim form
- Original official final and itemized medical bill(s) (including but not limited to) hospital tax invoices, attending physician bills, pre & post hospital tax invoices, medical/specialist reports and settlement letter from other insurers if bill(s) partially paid.
- For Restructured Hospital:
- Insured to complete page 1 and 2 of the Medical Claim Form
- Submit inpatient discharge summary
- Other supporting documents such as medical/specialist reports, settlement letter from other insurers if bill(s) partially paid.
Please note the above list of documents is not exhaustive. We will contact you directly if other documents are required.
Process claims
Upon receiving of Liberty’s medical claim form and all original copies of medical bills, we will proceed to process your claims in estimated 14 working days.
Payment
Upon approval, we will proceed to pay your claims in estimated 7 working days via GIRO.
Process claims
Upon receiving all original copies of pre and post medical bills with completed medical claim form, we will proceed to process your claims in estimated 14 working days.
Issuance of Letter of Agreement (LOG)
Alternatively, if you want to obtain LOG, you are required to submit pre-authorisation to Liberty. We will contact you in estimated 5 working days.
